The information required by this FBI form
is a result of the regulations mandated by the Public Health Security
and Bioterrorism Preparedness and Response Act of 2002 and is a component
of the security risk assessment. Unless exempted from registration
under the CDC regulations (42 CFR Part 73) or the APHIS regulations
(7 CFR Part 331 and 9 CFR Part 121), an individual or entity that
possesses, uses, or transfers select biological agents and toxins
must complete this FBI form for the entity, the responsible official
(RO), and alternate responsible official (ARO).
